Actually, both he and his brother succeeded their father. Their father and his several brothers succeeded their father, and so on back in time. Some dynasties do this, others do not. Although in the past it was practical for the successors to move to other centres, nowadays it is simply not practical since the haredi community is concentrated in a few large centres. However, the splitting up of a dynasty is usually the best thing that can happen to it, as they become so massive and unwieldy that no one individual can devote the necessary time to relating to his followers, and they simply lose touch.

What do rabbis know? They study worthless things, are not accustomed to life in our times. He suffered an infection at the end? That's quite remarkable at his age since he probably took showers rarely and was prone for an infection much earlier. Well, may he rest in peace.
