You should read the judge's opinion (and he quotes the Beit Din HaGadol, which also found the husband to be at fault and ordered a get.) The (haredi) wife sought a divorce in the first place in large part because her husband was compelling her to engage in halachically forbidden sexual acts. She wasn't looking for a 1/1000 man -- she was just looking for someone with whom to raise a family with yirat shama'im. And I doubt that hacking off her clitoris and sewing shut her vagina would solve her problem (which was not adultery or fantasy, but rather violent, illegal, and unwelcome sexual acts).
