US says blunders led to botched September strike in Syria

WASHINGTON -- A US military investigation has concluded a series of "unintentional human errors" led to a coalition air strike on Sept. 17 that killed at least 15 fighters aligned with the Syrian regime, believed at the time to be from ISIS.

Brigadier General Richard Coe, the investigating officer, told reporters that the errors ranged from misidentification of targets to "group think" during intelligence development and communication errors in a hotline with Russia.
