An internal Israel Railways report obtained by ynet revealed the chain of failures that led to damage to electrical infrastructure and transportation chaos. According to the preliminary report by the internal investigation committee, on Thursday a freight train departed from Israel Railways' sorting station in Ashdod heading north. The train operated with 20 flat cars, 18 of which were loaded, including ten cars with empty containers and two cars with loading platforms (pallets). Before departure, the car inspector testified that this was the first time he encountered this type of cargo, and that he "does not know the loading instructions and was not briefed on the matter." According to him, he informed the shift manager that he could not approve the cars, who replied that he would "take care of it" - but the freight train departed anyway, resulting in infrastructure damage and severe disruptions to train traffic.
