Many people love spicy food, but the heat may offer more than just an extra kick. A series of studies in recent years has found that people who regularly eat spicy foods tend to live longer and have lower rates of heart disease, certain cancers and lung disease. Researchers still cannot say that spicy food directly causes these benefits, but growing evidence suggests that capsaicin, the active compound in chili peppers, may have beneficial effects on the body.
One of the largest studies on the subject, published in the BMJ in 2015, followed more than half a million adults in China for about seven years. Researchers found that people who ate spicy food six to seven times a week had a 14% lower risk of death from any cause during the follow-up period compared with those who ate spicy food less than once a week. They also had a lower risk of dying from cancer, heart disease and lung disease.
Two years later, another U.S. study reached a similar conclusion, finding that consumption of hot red chili peppers was associated with a roughly 13% reduction in mortality risk. In 2020, a broader review analyzing four large studies involving more than half a million people found that chili pepper consumption was associated with 26% lower cardiovascular mortality, 23% lower cancer mortality and 25% lower all-cause mortality.
Other meta-analyses have supported the same trend, pointing to a 12% to 14% reduction in overall mortality and an 18% to 34% reduction in cardiovascular mortality. A 2025 study even linked spicy food consumption with slower biological aging.
Still, these studies show statistical associations only. They cannot establish that spicy food itself directly extends life or prevents disease.
How capsaicin affects the body
Capsaicin is the active compound responsible for chili peppers’ familiar heat. It works through cellular receptors called TRPV1, which detect heat and spiciness and are found throughout the body, including the skin, digestive system and immune cells.
Researchers believe capsaicin’s biological effects on these receptors may explain at least some of the health benefits seen in studies. Observational research has linked spicy food consumption to lower risks of heart disease and stroke, while small clinical studies suggest capsaicin may raise HDL, the so-called good cholesterol, and increase calorie burning.
Capsaicin may also improve blood vessel function, influence the gut microbiome, regulate metabolism and have strong anti-inflammatory and antioxidant properties.
A 2017 Chinese study involving about 600 adults found that people who preferred spicy food consumed an average of about half a teaspoon less salt per day and also had lower blood pressure. Another study by the same researchers found that capsaicin enhances the brain’s perception of saltiness, making the same amount of salt taste stronger and potentially allowing people to reduce sodium intake without sacrificing flavor.
Spicy food does not really ‘burn’ the stomach
Dr. Tal Engel, a gastroenterologist with Maccabi Healthcare Services and director of interventional clinical nutrition at Sheba Medical Center, says the findings on spicy food are indeed striking.
“The big secret lies in capsaicin, the active compound in the pepper,” he said. “It improves blood vessel function, has a positive effect on the microbiome, regulates metabolism and has clear anti-inflammatory and antioxidant properties. These are likely the mechanisms behind the impressive research data showing significant reductions in overall mortality and mortality from heart, lung and cancer diseases.”
One of the main concerns among people who enjoy spicy food, and those who avoid it, is whether it damages the esophagus or stomach. Engel says that is a longstanding myth.
“Contrary to popular belief, spicy food does not ‘burn’ the stomach or cause ulcers,” he said. “In ordinary dietary amounts, capsaicin may actually have a protective effect on the stomach: it reduces acid secretion, increases secretion of protective mucus and improves blood flow to the stomach lining. However, extreme amounts and excessive spiciness can impair intestinal permeability and cause severe abdominal pain and diarrhea.”
The burning sensation or heartburn that sometimes follows spicy food does not necessarily mean more stomach acid is being produced. Rather, it may simply reflect local activation of TRPV1 receptors in the esophagus, which translates contact with capsaicin into a sensation of heat.
Reflux, IBS and cancer
Engel notes that some groups need to be more cautious. People with reflux may experience worse heartburn after eating spicy food. Among people with irritable bowel syndrome, or IBS, capsaicin can trigger abdominal pain and bloating because of heightened sensitivity in the gut’s nervous system.
Still, he says there is no need for blanket avoidance. People should listen to their bodies and reduce spicy food during flare-ups if it worsens symptoms.
The connection between spicy food and gastrointestinal cancers is more complicated. Engel notes that a 2021 cohort study found a possible protective association with esophageal cancer, especially among non-smokers, while the evidence for stomach and colorectal cancer is weaker and requires further research.
Because most of the evidence comes from observational studies and self-reported eating habits, he stresses that the relationship remains associative rather than causal.
“My bottom line is simple,” Engel said. “If you enjoy spicy food, enjoy it, preferably in fresh, unprocessed form without preservatives. If it causes you discomfort, you certainly do not have to eat it. Similar health benefits can easily be achieved through a balanced Mediterranean diet.”
Small human studies also support the idea that TRPV1 receptors can adapt over time through a process known as desensitization. That means that even if spicy food initially causes discomfort, regular moderate intake over several weeks may reduce sensitivity and improve symptoms.
And contrary to another common concern, a randomized controlled trial published in 2006 found that capsaicin did not worsen symptoms in people with hemorrhoids.
Why chili peppers may help
Luda Nevo, a clinical dietitian with Maccabi Healthcare Services, says the findings should be viewed in perspective.
“With an endless list of articles, studies and dietary recommendations, it can be hard to take seriously the latest ‘superfood’ that is supposedly a magic cure for disease and mortality,” she said. “And yet chili peppers do have a very impressive body of research behind them.”
Nevo says the potential benefits are attributed not only to capsaicin but to the pepper as a whole. Chili peppers are also rich in vitamins, carotenoids and polyphenols, which may also benefit gut bacteria.
Still, she cautions that because the precise mechanism remains under investigation, it is too early to conclude that chili peppers alone are responsible for the outcomes seen in research.
She also points to a large Italian study in which the association between chili pepper consumption and lower mortality remained even after researchers adjusted for risk factors and overall diet quality, suggesting a possible independent effect. Even so, she notes that people who regularly eat spicy food may simply have healthier diets and lifestyles overall.
The healthiest way to eat spicy food
There is currently no broad clinical recommendation for a specific amount of spicy food people should eat, Nevo says. In studies that found lower mortality, “regular consumption” was often defined as eating spicy food at least once a week, so there is no evidence that daily intake is necessary.
Some findings also remain inconsistent. Nevo notes that certain studies have found an increased risk of stomach cancer among people who consumed spicy food very frequently and at very high intensity.
Another important factor is how the spicy food is consumed. Fresh chili peppers contain flavonoids, polyphenols, vitamins and minerals that may contribute to broad biological and anti-inflammatory effects. Chili powder retains many active compounds, although some vitamin content may fall during drying.
Commercial hot sauces and processed spicy foods are different. Many contain high levels of sodium, added sugar and preservatives, which may reduce or even cancel out the potential health advantages associated with chili peppers themselves.
For the healthiest approach, Nevo recommends choosing fresh whole peppers or minimally processed products and limiting industrial sauces high in salt and sugar. People who are not used to strong heat can start with milder peppers and gradually increase the intensity, while reducing intake if they experience gastrointestinal pain or discomfort.
